Application Procedure

Instructions to candidates for filling up the application form.
  • Ensure that the Demand Draft is for Rs.1500/- (One thousand Five hundred only) drawn in favour of The Registrar, BLDE University, Bijapur payable at Bijapur, Karnataka State.
  • The Online application form has to be filled up properly and completely.
  • After you have filled your Online application form, click on the submit button only once to send the details to the server. The server will assign you an application number, and provide the facility to view/print your online application form with the details you have filled, along with the application number.
  • Take a print of the filled in Application form and DOB Certificate / Identity Proof (Two Copies: Candidate's Copy and BLDE University Copy)
  • The completed application form must have the signature of the Candidate, left thumb impression and candidate's parent/guardian's signature.
  • Do not send any original documents along with the application form.
  • Affix your recent (not older than six months) passport size sharp and clear colour photograph only in the space provided. Application with smudged / tampered or hazy photograph will not be accepted. Do not staple or pin the photograph on the application form. Do not Sign and no attestation is required on the photograph.
  • Incomplete / incorrect applications are liable to be rejected without prior information to the candidate.
  • Any application received after the last date shall not be accepted.
  • The Application form is said to be complete only if it is enclosed with all the documents mentioned below and after verification of the same submitted in person or posted to: The Registrar, BLDE University, Smt. Bangaramma Sajjan Campus, Sholapur Road, Bijapur - 586103, Karnataka State, India.
  • Multiple application forms filled and received from the same candidate will result in disqualification.
  • Multiple Demand Drafts in excess of the prescribed fee (Rs.1500/-) if sent, by the candidate will not be refunded.
  • Demand Draft for Rs 1500/- drawn in favour of The Registrar, BLDE University, Bijapur payable at Bijapur, Karnataka State. (mention your application number on the back of DD). Check for the banker's signature on the DD.
  • BLDE University Copy of Date of Birth Certificate / Identity Proof in the prescribed form duly attested by the head of the institution where he/she has studied last or Gazetted officer from the place of residence of the candidate.
  • Additional photograph (mention your application number on the back of the photograph)
  • Please note the Photographs to be provided are identical, sharp, clear in Colour and clarity, and should not be more than 6 months old. Application with smudged / tampered or hazy photograph will not be accepted. Submit multiple copies of same photograph and not of different photographs. Any application received not adhering to these specifications is liable to be rejected without any clarifications and no refund will be made of the application money. The decision of the University on this would be final and binding.
  • For all future correspondence with BLDEUGET - 2008, applicants need to quote their Application Number and TAT (Test Admission Ticket) Number (if applicable).
  • Brochure and application form can be down loaded only through our website www.bldeuget.org
  • Candidates are advised to visit our website www.bldeuget.org regularly to watch for "Red Alert" list of incomplete applications to ensure corrections well in time and to enable BLDE University to send the Test Admission Ticket (TAT) in time. BLDE University is not responsible for rejection if clarifications listed in the red alert are not provided in time
  • The completed Application Form has to reach The Registrar, BLDE University, Smt. Bangaramma Sajjan Campus, Sholapur Road, Bijapur. either in person / post / courier, on or before 5:00 pm of 26th April 2008.


Name of the Applicant:
Write your full Name (as it appears in your School Leaving Certificate - HSC/SSLC/10th), your name should not exceed 30 characters including the blank spaces. Should your full name exceed 30 characters, and does not fit in the space provided, please shorten your name by using initials. Please use a blank space between initials. Do not use any titles like Mr., Miss/Ms. etc.

Date of Birth:
Provide your date of birth in DD/MM/YYYY format only. For e.g.: If your date of birth is 10th October 1990, fill as 10/10/1990

Choice of City/Town:
Select the city / town of your choice, where you wish to take up your entrance test.
For e.g.: If you wish to take up the test in Bijapur, select Bijapur from the list of test centres.

Gender:
Select the appropriate gender.

Nationality:
Please mention your nationality.

By Birth/Domicile:
If you belong to Karnataka by birth / domicile, please select Karnataka.

Category if any:
Enter the Category you belong to.

For Karnataka candidates:
Select the category you belong to: General, SC, ST, OBC Cat.1, OBC Cat.2A, OBC Cat.2B,, OBC Cat.3A,, OBC Cat.3B (Applicable to Karnataka candidates only)

For Non - Karnataka candidates:
Select the category you belong to: General, SC, ST (Applicable to Non-Karnataka candidates only)

Address:
Provide your address for communication. Note that all formal communications from BLDE University will be sent to you at this address only. Please use blank spaces to separate words. Avoid short form such as Rd, Th, St etc. Avoid Roman letters in your address. Specify the name of your city, state or pin code in the columns provided only; do not mention your city name in the space provided for address.

City:
Kindly select the city from the list shown. If you are from any other city that is not listed in the table, select "Others" in the list provided, and fill in your name of city in the space provided

State:
Select the state from the table

PIN Code:
Fill up with the appropriate 6-digit PIN code.

PAYMENT DETAILS:
DD No.:
 Fill up the six digit DD number given in the bottom of the demand draft.

Drawee Bank:
Fill up the name of the bank from where the DD was purchased in the space provided.
Ensure that the DD is signed by the Bank authorities.

Name of the Institute Last Studied:
Fill up the name of the Institution you have last studied in the space provided.

Place of the Institution:
Fill up the Place of the Institution last Studied in the space provided.

Year of Passing:
Fill up the Year of Passing in the space provided.

Phone No.:
Kindly fill your Phone number with the relevant STD Code, use zero prefixed to the STD code and do not leave any blank spaces between the STD code and your telephone number. Unless you enter the phone number the application form cannot be downloaded. For e.g.: If your STD code is 040 and your telephone number is 12345678, then your full telephone number will be "04012345678"
